Ambient AI clinical scribe for telehealth

From visit to chart, automatically.

Visit to Chart listens during your telehealth visit — with your patient's consent — and drafts a structured, specialty-specific clinical note. You review, edit, and sign. The note stays yours; the typing doesn't.

Visit to Chart is currently in invitation-only early access. Contact us to request access.

How it works

Four steps from conversation to chart

Visit to Chart runs quietly alongside the telehealth platform you already use. No new video software, no change to how you practice.

Consent, then record

Patient consent is captured at the start of the visit. Only then does recording begin.

Medical-grade transcription

The conversation is transcribed with a speech-to-text model built for clinical vocabulary.

A structured draft note

VTC drafts a note in your specialty's format — history, assessment, and plan organized the way you'd write them.

You review, edit, sign

The draft comes to you for review. You edit what needs editing, sign, and export the finished note to your EHR.

The note lands minutes after the visit ends — and it is built to handle long visits, not just short ones.

Built for your specialty

Your specialty's notes, in your specialty's language

Every specialty documents differently. VTC drafts notes in the structure, terminology, and level of detail your field expects — from intake evaluations to follow-ups to session-specific formats.

Psychiatry & behavioral health

Intakes, medication management, and therapy-session progress notes.

Family medicine

From well visits to chronic care, in the format your day demands.

In development Coming soon

Internal medicine, endocrinology, and sleep medicine are in development and not yet available to practices.

Your specialty

Adding a specialty is configuration, not a rebuild — tell us what you practice →

Under the hood, each specialty is a configuration, not a separate product — note formats, terminology, and safety rules are tuned per specialty on the same foundation. Sensitive documentation (for example, risk assessment in behavioral health) is handled under strict safety rules designed to keep the note to what was actually said in the visit.

Safety & trust

You are always the author of record

An AI scribe only earns a place in your practice if you can trust what it writes — and verify what it doesn't.

You sign every note

The physician reviews, edits, and signs every note before it goes anywhere. Every note carries a clear AI-assistance disclosure.

Designed not to invent findings

Exams, vitals, labs, and risk language are drawn from what was said in the visit. Anything that wasn't said is flagged for you to complete rather than filled in.

Every note is cross-checked

A second AI pass compares each draft against the transcript and flags any statement it can't support, so you see exactly what to verify.

Consent comes first

Patient consent is captured before any recording starts. No consent, no recording — it's built into the workflow, not left to memory.

Security & privacy

Built for HIPAA compliance

Visit to Chart follows HIPAA-compliant practices, and protected health information is handled as if every note were about someone you know.

Encrypted in transit and at restEverywhere your data lives, on the way there and once it arrives.

Per-clinician isolationEach clinician's data is kept separate from every other's.

Notes don't lingerNotes are deleted when you export them to your EHR, and auto-purge within 7 days either way.

Audit loggingNote, access, and lifecycle activity is logged and reviewable.

Multi-factor authenticationMFA on every account, no exceptions.

BAA availableA Business Associate Agreement is available for your practice.

About

Built around how clinicians actually document

Visit to Chart is built by Hire Wisely LLC. It exists because documentation was eating the part of medicine that matters — the visit itself — and because independent and small practices deserve the same ambient documentation tools the big systems get.
